Those that wish to make application to the Fund, must meet these minimum requirements:
i. Received a cancer diagnoses from a qualified and licensed oncologist within the last 30 days. Types of cancers currently supported include Non-Hodgkin’s Lymphoma, Hodgkin’s Lymphoma and Leukemia.
ii. Provide a letter from your oncologist or social worker supporting the diagnosis. The letter must include your written permission and your signature (see sample letters).
iii. Successfully complete the Fund Application Process.
iv. Other qualifications may apply at our discretion.
