All vendors that wish to make application to our Approved Vendor Program must meet the following minimum qualifications:
i. Have one of the following business entity designations:
1. Incorporation
2. Limited Liability Corporation
3. Professional Corporation
4. Limited Partnership
5. Limited Liability Partnership
6. Non Profit Corporation
ii. Provide proof of business operation for a minimum of one year
iii. Provide proof of general liability insurance
iv. Sign a Statement of Understanding (SOU)
v. Sign a Confidentiality Statement
vi. Other qualifications may apply at our discretion
